In-Network or Out-of-Network?

What exactly does it mean to have an in-network doctor when the procedures you are seeking aren’t covered by insurance? Unless you are lucky enough to have insurance that covers fertility treatment, insurance usually only covers the diagnostic work-up. Patients soon learn that the costly treatments are out-of-pocket whether the doctor is in network or not.
